Immunization Requirements and Shot Clinics

Texas state law requires proof of current immunization for students to remain in school.

_____Diphtheria/Pertussis/Tetanus ~ (PreK-5th ) 3-5 doses. Children under the age of 7 are required to have at least four doses with the last dose on or after the 4th birthday. Older children may have 3 doses with one dose on or after the 4th birthday.

_____Polio ~ (PreK-5th) At least 3 doses with 3rd dose on or after the 4th birthday

_____Measles, Mumps, and Rubella ~ (K-5th) 2 doses with first dose on or after the 1st birthday.Pre-K requires 1 dose of MMR on or after 1st birthday.

_____Hepatitis B ~ (PreK-5th) 3 doses

_____Varicella (chickenpox) ~ (K-5th) 2 doses are required. The first dose must be given on or after the 1st birthday. Pre-K requires one dose. Written documentation of illness is acceptable proof of immunity.

_____Hepatitis A ~ (PreK-5th) 2 doses with the first dose on or after the 1st birthday.


Early Childhood and Preschool Students

_____Hib CV ~ (Haemophilus influenza type B conjugated vaccine) EC and Pre-K students- 3 doses with last dose on or after 1st birthday. If 1st dose is started after age 1, then only 1 additional dose is required (2 doses total)- If 1 single dose on or after 15 months of age, then compliant with 1 dose

_____PCV ~ (Pneumococcal) ~ EC and Pre-K students - 3-4 doses…3 doses with one dose on or after 1st birthday/or 2 doses with both on or after 1st birthday/ or 1 dose on or after 2nd birthday… If not then 1 additional dose is required.

_____TB ~ (Mantoux, intradermal) ~ students who have resided outside the US within the last year and who respond affirmatively to the HCPHES tuberculosis screening questionnaire are required to have a TB test administered in the US with a validated result within the last 12 months. Positive reactions are required to be followed by a chest x-ray.

Exclusions from Compliance ~Exclusions from compliance are allowable on an individual basis for medical contraindications and Conscientious Objection. Students in these categories must submit evidence for exclusion from compliance as specified in the Texas. Adm Code, Title 25, Chapter 97.63.
_____Medical Exemptions ~ the student must present an affidavit or certificate signed by a physician, duly registered and licensed to practice medicine in the United States, in which it is stated that in the physician’s opinion, the immunization required would be injurious to the health and well-being of the applicant or any member of his or her family or household. Unless a life long condition is specified, the affidavit or certificate is only valid for one year.

Immunizations may be obtained from private physicians or from Harris County Clinics. Always bring your child’s immunization record to your visit. Please call first to verify availability of the vaccine, appointment times, and cost.
